Please follow these steps:
- Download the necessary application form(s):
- Print the form(s).
- Fill out the form(s) accurately and completely.
- Get form(s) notarized.
- Gather all necessary paperwork for submission as a packet:
- Form(s) filled out, signed and notarized
- Board of Health Permit to Operate
- Fire Marshal Permit (if applicable)
- Proof of Newspaper Ad in local newspaper
- Scan all documents and email to taxinfo@lpso.net.
- Alternatively, you can drop documents off at any LPSO office or mail to: Lafourche Parish Sheriff’s Office, P.O. Box 5608, Thibodaux, LA 70302
Once application(s) are received, LPSO completes a background check on the applicant(s). If no criminal history is found, the application is sent to Lafourche Parish Government for approval. Once approved by the Parish President, Lafourche Parish Sheriff’s Office issues the license. This process takes approximately two weeks if no criminal history is found. The Sheriff’s Office will then contact the applicant(s) by email with information on how to pay for the license. The fees are as follows:
- Beer $50 ($25 after July 1)
- Liquor $200 ($100 after July 1)
- Beer/Liquor Combo $250 ($125 after July 1)
